Please understand that your child will NOT be assigned to a team until a valid birth certificate is received and approved by the PAYSL Registrar along with payment due. Team formation will begin at the close of registration on 10-10-08 at 5pm.  No child will be placed on a team before payment and birth certificate are received. 

There are 2 ways to submit a birth certificate that will be uploaded by the Registrar to your player registration:

1. Fax it to the following toll-free fax number: 1-866-760-8476
2. Scan the birth certificate and email it to: payslbc@yahoo.com
Please do not send anything but birth certificates to the above email address.
